Services include exercise and fitness programming; integrative therapies including massage, acupuncture and water shiatsu; education and social support groups; stress reduction training; and nutrition assessments and counseling.…women have access to convenient and coordinated healthcareThe Mayora Rosenberg Women’s Health Center, which opened in Fall 2014, removes barriers that many women in our community face when accessing healthcare. Since opening, more than 13,000 women have accessed services in the state-of-the-art facility.…uninsured patients experiencing a psychiatric crisis receive timely emergency careIn partnership with LSSI/Project IMPACT, Swedish Covenant Hospital houses a psychiatric advanced practice nurse in the emergency department to provide expedited care to uninsured patients who present in an acute behavioral health crisis.… patients at high risk of readmission receive the support they need as they transition homeOur Transitional Care Program, an evidence-based intervention, serves adults who are at high risk of readmission following a hospital stay. The program delivers disease management education, home visits, home tele-health monitoring and care coordination.…patients have access to today’s technology for improved rehabilitation outcomesPatients on inpatient rehabilitation units have access to rehabilitation technology that has been proven to improve attention, concentration, use of affected upper extremity from stroke and assist with standing endurance.…new mothers receive lactation support in Chicago’s only outpatient clinicThe Lactation Support Clinic provides new mothers with post-discharge lactation support vital to the successful continuation of breastfeeding and advances Swedish Covenant Hospital’s progress toward Baby-Friendly designation.…patients and their families heal through artThe Artist-in-Residence program provides structured and scheduled art opportunities for patients with the therapy team on the rehab and extended care units, the adult mental health unit, and the cancer center. The program also provides bedside opportunities upon request.…geriatric patients are cognitively engaged using activity cartsThe use of patient activity carts throughout the hospital units provides therapeutic stimulation to elderly patients with cognitive impairment, such as delirium and dementia.…victims of domestic violence, sexual assault and human trafficking receive the supportive care they needA new program at Swedish Covenant Hospital was created to strengthen the hospital’s capacity to identify and respond to victims of domestic violence, sexual assault and human trafficking.
