Swedish Hospital is committed to providing high-quality care for our diverse community, and with your help, we can continue to care for thousands of patients each year. A gift to Swedish Hospital Foundation will support an array of hospital initiatives and programs designed to provide the best health care experience for our patients and their families. There are a variety of ways to provide assistance, including:
Monetary
Gifts by cash, check or credit card may be mailed along with a donation form to Swedish Hospital Foundation, 5145 N. California Ave., Chicago, IL 60625.
Online gifts may be made using MasterCard, Visa or Discover, and processed through our secure payment site.
Stock gifts may be transferred to Swedish Hospital Foundation directly from your broker. Please contact the foundation office for stock transfer instructions at (773) 293-5121 or schfoundation@schosp.org.
In-Kind
Donations of non-cash gifts (including goods, services, time and expertise) are accepted depending on the purpose and needs of the selected program or department.

Tribute/Memorial Gifts
Making a memorial or tribute gift is a wonderful way to honor someone special while supporting the mission of the hospital. The Tree of Life, located on the hospital’s third floor, offers engraved leaves, apples and doves to honor loved ones or memorialize life’s special events.

Planned Giving
Many donors have enhanced the hospital’s ability to care for the community while successfully increasing the size of their estates and/or reducing the tax burdens on their heirs. Such as a bequest or charitable gift annuity are excellent ways to leave a legacy while supporting a program or service at Swedish Hospital. Please contact the foundation office or visit the Planned Giving page for more information. In addition, friends who provide a planned gift are recognized as members of the hospital’s Heritage Society, and invited to special programs and events throughout the year.
