Community CARE (Creating Access to Resources for Equity)
Swedish Hospital offers a wealth of medical and wellness services and operates several community programs. Swedish Hospital's Community CARE (Creating Access to Resources for Equity) includes a group of programs with the mission of honoring the human dignity of all, while building connections that expand the reach and definition of healthcare. These programs collaborate and can provide support internally and through a cadre of external community partners to address multiple needs of patients and community members beyond their traditional healthcare.
Programs include:
Community Behavioral Health: decreasing stigmas around mental health, increasing access to group and individual mental health services, and training for providers and community members
Community Nutrition: community-facing nutrition programs, including Food Connections connecting food-insecure patients with additional resources
Healthcare Transformation Program: Embedding specialty care in partner Federally Qualified Health Centers so that community can access this care with fewer barriers
Housing Connections: Connecting houseless high-utilizers of the emergency department with permanent housing and supportive services
Maternal Health: provides support, guidance and new-born care for families who give birth and live in Chicago.
Pathways: providing survivors of domestic violence, sexual assault and human trafficking with crisis intervention and connection to long-term resources
